    The MetroHealth Community Health Centers Community Outreach Team is looking for an APP to support our growing programs. We are seeking a provider who can provide care to patients across the lifespan with primary assignment being within the School Health Program. Community Outreach within the FQHC-LAL continues to grow, and your clinical location may change daily (schedule known in advance) at school sites in an in-school clinic or mobile unit. This APP will serve patients age 3 and up, offering comprehensive primary care, telehealth services, chronic disease management, and care team.     Family Medicine certification and experience preferred

    1-2 years of APRN experience required

    Qualifications:

    Required: MSN or grandfathered under Ohio Revised Code Graduate of an accredited advanced practice registered nurse program. Current registered nurse licensure in the state of Ohio Certificate of Authority as an Advanced Practice Nurse from the Ohio Board of Nursing Certification as Advanced Practice Registered Nurse in specialty. Current Basic Life Support (BLS) from the American Heart Association or obtains within 90 days of hire. Obtains other advanced life support certifications applicable to practice area within 180 days of hire. Meets the requirements/qualifications for clinical privileges in the specialty/department with The MetroHealth System and must report to Medical Staff Office at time of hire to apply for clinical privileges. Maintains a standard care arrangement and renews/signs it as required by the Ohio Board of Nursing Note: Other department specific qualifications for providers will be addressed through the Certificate of Authority and credentialing process. Preferred: Have or be able to apply for a Certificate to Prescribe/Externship from the Ohio Board of Nursing. PA's & APRN's accepted.
