  • Purpose:

    The Community Aide-I OP assess, plan, and implement care for adult individuals with serious persistent mental illness who reside in long term care. Assist with meal preparation, serving and clean up. Assist residents with activities of daily living.

    We are a 24/7 residential program working with adult men and women who have mental health concerns. They need assistance transition back into the community. Staff is responsible for ensuring safety, assisting with ADLs, meals, outing, shopping, and participating in activities. This position works a rotating shift which includes variable 12-hour shifts and one 8-hours shift.

    Responsibilities:

    • Leads/co-leads living skills groups, community and wrap up meetings and leisure/recreational activities with supervision.

    • Supervise and assist residents with activities of daily living (ADL) for all clients at the LTSR.

    • Document information pertinent to residents such as: observation of behavior, interventions used and resident response to treatment in the medical record. Complete other documentation as assigned.

    • Implement the milieu program and/or individual skills training programs under the direction of the program Coordinator or designee.

    • Participate in change of shift report addressing each resident's activities and report all pertinent information to the next shift.

    • Maintains a safe and therapeutic environment at all times.

    • Communicate pertinent information to the team regarding resident behavior, respond to treatment and level of functioning in terms of activities of daily living skills.

    • High school education or equivalent.

    • 1 year of experience in the care of mentally ill individuals or equivalent mental health experience.

    • Able to work with clients and their families under the direction of the treatment team in collaboration with other agencies.

    • Able to make decisions quickly under stressful conditions and to prevent minor incidents from escalating into major problems.

    • Able to work evenings, nights and weekends as program needs dictate.

    • Able to communicate effectively in English both orally and in writing.

    • May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients. Transportation of medication is limited from one department to another.

    Licensure, Certifications, and Clearances:

    CCM training is required within 30 days of hire and annually thereafter.

    • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

    • Comprehensive Crisis Management (CCMC)

    • Driver's License

    • Act 34
