    Description:

    Work late evenings including weekends to screen patients in the ED for substance use and mental health. Partner with community-based organizations to connect these patients to resources after discharge.

    :

    With combined experience and knowledge on how to navigate the healthcare system, the CHW will be the trusted messenger to ensure a timely connection to resources to support patients who have been screened for substance use disorder, non-fatal opioid and mental health. The role will ensure there is connection to a primary care provider, assist with appointments to support groups, develop a wraparound care plan to prevent patients from readmission to ED for same conditions.

    Individual should have lived experience with mental health and substance use.

    Schedule and Pay:

    • Part time 20hrs/week - late evenings including weekends. Typically 7p-11p but must be flexible to meet the needs of the clients served in the Emergency Department. $22/hr PLUS DIFFERENTIALS for working evenings and weekends.

    Requirements :

    • Minimum High School diploma or equivalent

    • Community health worker training and/or certification preferred

    • Comprehensive knowledge of the local community

    • Bilingual Spanish preferred
