    Position Summary: The Tidelands Health Hospital Medicine Education Coordinator coordinates and implements patient education programs for hospitalists and the collection of relevant data to measure physicians for Quarterly Business Reviews. Develops a comprehensive evidenced based care patient education program based on the identified needs of the organization through evaluation of current education platforms (Micromedex, TiGR, and etc.) and makes recommendations on the platforms, programs, improvements needed to expand the program. Remains abreast of changes in the education needs for specific disease states through collaboration with Hospitalist medical team, consultation with professional patient education organizations, and consultation with the manager of Quality and Evidenced-Based Care for Tidelands Health. Ensure that hospital medicine operations and providers are knowledgeable of and in compliance with applicable regulatory, licensing and accrediting agency standards and applicable state and federal laws. Manages the education process for nursing/ancillary staff regarding patient education and coordinates education efforts with the appropriate unit director. Involvement in the development of process improvement opportunities by identifying high-risk patients with perceived learning deficits to develop education plan with patient, PSP, and nursing staff, including high-risk readmission patients.

    Assists the CSC Director with various administrative aspects of the daily operation of the Hospital Medicine CSC including training; policy & procedure management; medical record management; technical issues; daily flow and evaluating standards of care for patients.

    QUALIFICATIONS

    Education & Experience:

    • Associate's degree in healthcare or business related fields with five (5) years prior experience working in an acute care clinical setting

    -OR-

    • Bachelors degree in healthcare or business related fields with three (3) years prior experience working in an acute care clinical setting

    - Demonstrated experience leading patient and/or employee education, training and competencies

    • Experience in education program design and evaluation

    Knowledge/Skills/Abilities:

    • Must be strongly proficient with PC's and Microsoft office software such as Word, Excel, PowerPoint, and Outlook required.

    - Prior Electronic Medical Record experience required.

    - Strong analytical problems solving skills required.

    • Exemplary communication and teamwork skills required.

    Physical Requirements: Light Physical Agility Test (PAT) Rating
