    Job Summary:

    Monitors and maintains regular contact with patients as identified by their physician. Performs pre-visit chart reviews and coordinates patient visits, consults, labs, x-rays, ancillary services, and community resources. Provides additional staffing support in the clinic, as needed.

    Job Responsibilities and Requirements:

    PRIMARY RESPONSIBILITIES

    • Serves as a liaison for management staff, administration, physicians, managed care companies, and other customers.

    • Monitors and maintains regular contact with patients in the high-risk caseload as identified by physicians. Consults with a physician to determine the frequency of contact according to the patient's situation at the time.

    • Performs a pre-visit chart review on the high-risk caseload patients. Identifies labs to be done, prescriptions to be refilled, consultations that may be due, etc. and ascertains that all appropriate preventive care is being performed.

    • Coordinates outreach processes for the patients with care opportunities identified by a physician, including visits, consults, lab, x-rays, ancillary services, and appropriate community resources.

    • Serves as point of contact for all patients within the service to support discharge plan; medication review and schedule follow up appointments with the provider.

    • Educates patients about self-management tasks they can undertake to gain greater control of their health status and coordinates with physician and care team workflows to involve the patients in activities to improve their health.

    • Works with the practice leadership to implement evidence-based practice guidelines as outlined by clinic policies and providers.

    • Provides emergent staffing needs, e.g. room patients, schedule appointments, document patient encounters in EHR, etc. as directed.

    • Works in a constant state of alertness and safe manner.

    • Performs other duties as assigned.

    EDUCATION

    • Graduate of an accredited school of nursing or education equivalency for licensing

    EXPERIENCE

    • Two years licensed practical nurse experience

    PHYSICAL REQUIREMENTS

    • Constant use of speech to share information through oral communication.

    • Constant standing and walking.

    • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.

    • Frequent sitting, reaching and keyboard use/data entry.

    • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.

    • Frequent use of smell to detect/recognize odors.

    • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.

    • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.

    • Occasional lifting/moving of patients.

    • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.

    • Occasional driving.

    • Rare crawling and running.
