Pay Rate: $65.00-$73.91 per hour Responsibilities: As a Nurse Practitioner at Snowline Hospice, you will be responsible for providing compassionate and comprehensive care to our patients and their families. Our Supportive Care Nurse Practitioner plays an important role in the care journey for our patients and families! As our SC Nurse Practitioner, you will collaborate with our Medical Director and other care team disciplines to provide support to our care programs in all 3 counties that Snowline serves. Snowline's clinicians are experts in providing patients relief from the symptoms, pain, and anxiety while walking patients and caregivers through the difficult and often challenging healthcare systems. They guide patients and caregivers through difficult and complex treatment choices; and providing emotional and spiritual support to patients and their families. Your duties will include:
    • Working in collaboration with the Medical Director: function as a consultant, clinician and mentor in the delivery and evaluation of hospice, palliative care and pain management services
    • Works as part of the interdisciplinary team, collaborates and communicates with other physicians and providers as needed
    • Performs bedside patient assessments and assists with diagnosis, treatment, and management of acute and chronic health conditions
    • Orders, performs and interprets laboratory and radiology tests within scope of professional practice
    • Prescribes medications including controlled substances to the extent delegated and licensed and orders treatments and durable medical equipment as indicated
    • Performs other therapeutic or corrective measures as indicated, including urgent care
    • Assists in all facets of care coordination for palliative care and hospice referrals
    • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
    • Complies with applicable laws and regulations with respect to Collaborative Agreements
    • Regularly participates in meetings, coverage, after-hours on-call rotation, training, and education
    • Follows applicable policies, procedures, regulatory requirements and laws
    • Adheres to behavioral values and conducts themselves professionally at all times

    Your Experience and Education:

    • Our ideal candidate will have a minimum of 2 years of experience or more in direct Hospice/Palliative Care and be skilled in the following areas: Symptom Management, Goals of Care Discussions, Advance Care Planning, Pain Management/Relief as achieved by at least five years of experience.
    • Palliative Care certification is preferred but not required.
    • Graduate of a Nurse Practitioner Program approved by the Board of Registered Nursing, an unrestricted Nurse Practitioner License, Current Drug Enforcement Agency (DEA) Certificate, Furnishing number issued by the Board of Registered Nursing
    • Graduate from an accredited School of Nursing
    • Licensed Registered Nurse in the State of California
    • Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Association - American Nurses Credentialing Center (ANCC) and the State of California
    • Controlled substance furnishing number with a current DEA number
    • Eligible or currently possess a Medicare/MediCal UPIN number
    • Eligible to obtain hospital privileges
    • Excellent oral communication skills, experience working in a collaborative care team environment and functioning autonomously with minimal supervision
    • Excellent observation, problem solving, verbal and written communications skills
    • Demonstrated ability to efficiently use a computer, prior electronic health record (EHR) experience preferred
    • Current CPR certification
    • Valid CA driver's license, with a reliable vehicle that is insured in accordance with state requirements CPR certification
