Job Description:

The Outpatient Specialty Pharmacists are trained on the specialty medications dispensed and medical conditions encountered in the unique patient population serviced. The position entails patient care and requires frequent use of clinical knowledge and discretion due to the complexity of the therapies and conditions they treat. The Outpatient Specialty Pharmacists are responsible for adherence to all Specialty Pharmacy patient care policies to ensure compliance with various required regulatory and accreditation patient care outcomes e.g. Risk Evaluation and Mitigation Strategy (REMS), patient education/compliance, disease state management and other outcomes. Unique aspects of this role include pharmacovigilance reporting, acting as clinical subject matter expert for disease states associated with specialty medications, and support Kaleida Health's practitioners inclusive of pharmacists, providers, nurses and other internal/external stakeholders.

Minimum or Preferred Qualifications:

BS in Pharmacy required. PHARMD - Doctor of Pharmacy preferred. 1 year of experience working as a pharmacist or pharmacy intern capacity required. 2 years of experience dispensing complex medications as a pharmacist in an institutional or specialty pharmacy preferred. Pharmacist required upon hire. Board of Pharmacy Specialties within 1 year required. PGY-1 Post Graduate Year Residency preferred.

$95,503-$149,366 annually Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
