  • Responsibilities

    The Care Navigator LVN provides direct nursing care to an assigned group of patients, under the appropriate supervision of a Provider, Registered Nurse, by applying clinical nursing knowledge, skills, and ensuring the safety and comfort of patients and families according to legal, organizational and professional standards.

    The Care Navigator LVN prioritizes and makes judgments concerning basic needs of multiple patients in order to organize care. Assists with outreach initiatives and goals set forth by the Population Health leadership. Ongoing liaison between patient, multispecialty care departments and approved Facilities.

    Salary Range: $24.52 - $36.78 (Pay is based on several factors including but not limited to education, work experience, certifications, etc.)

    Qualifications

    Education

    Required: Graduate of an accredited LVN program

    Preferred: N/A

    Experience

    Required: 3-5 years' clinical LVN experience in an outpatient or acute care setting

    Preferred: Care Management / Disease Management experience

    License(s)

    Required: Current LVN License, License Vocational Nurse in the State of Texas.

    Preferred: N/A

    Special Skills

    Required: Current BCLS

    Preferred: N/A

    Other

    Required: Position requires sporadic travel throughout the greater Houston area to facilities and patient

    homes.

    Must have reliable personal transportation and current insurance coverage

    Preferred: EPIC experience Windows based computer skills
