Pharmacist Inpatient - Clin II - Critical Care/Emergency Medicine

Responsibilities

General Purpose

Critical Care Clinical staff pharmacists are responsible for the provision of comprehensive clinical pharmacy services to optimize safe and effective drug therapy. Clinical activities include participation as a member of an inter-disciplinary patient care team in the intensive care unit, order verification, pharmacokinetic drug monitoring, code response, and drug information. The Critical Care and Emergency Medicine pharmacists work together on the same team and are expected to cross train and cross cover as departmental needs necessitate. The clinical staff pharmacist will also have a working knowledge of the operational aspects of the department and will assist with dispensing of medications and technician oversight when applicable.

Specific Responsibilities

• Order Verification: Assess orders for clinical appropriateness. Collaborate with providers to clarify as needed. Facilitate timely approval and distribution to patient care areas.

• Bedside rounds: Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to guide and recommend appropriate medication use. Attend daily medical rounds on the intensive care units.

• Pharmacy consultation, clinical initiatives: Perform daily pharmacokinetic monitoring on medications per institution policy. Assess patients' renal function and medication profile for necessary changes to therapy. Review laboratory findings for appropriateness of anticoagulation regimens.

• Clinical Inquires/Drug information: Provide timely response to providers' inquiries for information on medication related questions. Perform literature search and review findings for applicability to patients within the institution while adhering to institution policy, procedures and guidelines.

• Problem solving: Thoroughly resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner. Proactive in identifying patient care problems or safety concerns. Contacts supervisor for assistance when solution is beyond his/her expertise or authority.

• Code Response: Attend cardiac arrest response within the institution. Provide pharmaceutical assistance and medication preparation as needed for stroke response within the institution. Facilitate medication distribution and provide clinical consultation and Pulmonary Embolism Response and Sepsis responses within the institution.

• Other Duties as assigned: Medication dispensing/checking: Assist with ensuring timely and accurate medication needing pharmacist review prior to dispensing.

• Training and Education: Provides training, mentorship and feedback for evaluating residents and other trainees as needed. Contributes to the training of pharmacy students for IPPE and APPE rotations.

Qualifications

• Minimum education required: Bachelor of Science degree in Pharmacy or Pharm D

• Minimum experience required: Either prior (>1 year) relevant acute care pharmacy experience or has successfully completed an accredited pharmacy practice residency)

• New York State Pharmacy License or eligibility required

• Training in hospital pharmacy practice, including internship preferred

Pay Range

Pay Range: $107,910 - $161,907 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
