    Description

    Physical Therapist in a Long Term Care Facility in Abilene, TX

    13- Week Contract starting 4/22/2024 $53.00 per hour 8-hours days. No weekend and no on-call. Send your resume and other info even if you can't start on the 22nd but could start sometime in the next month.

    The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient's ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability and work with the individual to prevent the loss of mobility.

    Essential Functions and Responsibilities

    1. Performs patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data.

    2. Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders.

    3. Conducts ongoing reassessment of the patient and their response to treatment.

    4. Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.

    5. Supervises and delegates to ancillary staff and students consistent with state laws and department policy.

    6. Completes legible, accurate and clear documentation within the established timeframe identified in the department policy and procedure and in accordance with accreditation requirements and insurance regulations.

    7. Communicates with the patient, family, caregivers, significant others, and members of the health care team to promote maximum benefit of care utilizing AIDET.

    8. Assesses educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs.

    9. Participates in professional growth and development through participation in educational programs, including in-service meetings, workshops, and by reading current literature concerning scope of practice.

    10. Actively participates in quality improvement activities.

    11. Provides an environment conducive to safety for patients, visitors, and staff. Complies with appropriate and approved safety standards.

    12. Functions within scope of practice as defined by the state's practice act.

    13. Performs other related duties as required and directed.

    14. 2 years PT experience - Required

    15. TX or compact license - Required

    Position Requirements

    Requirements:

    1. Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.

    2. Valid state licensure or eligibility for licensure as a Physical Therapist.

    3. Previous experience working with geriatric population preferred, especially in a long-term care or skilled nursing facility setting.

    4. Strong clinical assessment and treatment skills, including knowledge of therapeutic exercise, manual therapy, and modalities.

    5. Excellent communication and interpersonal skills, with the ability to collaborate effectively with patients, families, and interdisciplinary team members.

    6. Proficiency in using electronic medical record (EMR) systems for documentation.

    7. Ability to prioritize workload and manage time efficiently in a fast-paced environment.

    8. Commitment to professional growth and development through ongoing education and training.

    9. CPR certification (may be required depending on facility policies).

    10. Physical stamina and dexterity to perform job duties, including standing, bending, lifting, and transferring patients as needed.
