    The Pharmacy Team within MSHP drives the implementation and ongoing improvement of initiatives that enable Mount Sinai to deliver better value to its patients, its customers (i.e., plan sponsors and payers), its providers, and its partners. The Pharmacy team works directly with the Chief Population Health Officer and Chief Medical Officer of the Clinically Integrated Network of Mount Sinai Health System.

    Role Summary

    Mount Sinai is embracing new payment models that reward value-based arrangements to foster ongoing health improvement. The Pharmacy team is responsible for developing programs, measurements and processes to improve the overall total cost of care, ensuring that we meet critical contract thresholds and that our patients have an effective and seamlessly connected healthcare experience.

    The Program Manager for Pharmacy role is both strategic and operational, working directly with clinical and operational leadership to develop strategy, identify and track performance on key indicators and cost indicators, analyze performance against national benchmarks, plan content for physician education initiatives, and serve as the point of contact for the population health team and physician practice managers. The Program Manager will interact with a wide variety of stakeholders throughout the health system and prepare executive presentations about the program's financial and clinical impact.
