    Job Summary :

    The Psychosocial Patient Navigator / Clinical Therapist's role in the care of oncology patients includes an understanding of and participation in the treatment of patients following established standards as outlined by departmental policy and procedural manuals and professional licensing agencies. The social worker is part of a multi-disciplinary team servicing the patients and their families. The social worker is responsible for the evaluation, planning, provision of resources, and support of biopsychosocial services and interventions to improve quality of life and navigation of the cancer continuum for patients and their families/caregivers. Duties of the social worker include, but are not limited to, assessment of biopsychosocial and practical needs of patients through screening tools, interventions to assist in adjustment to diagnosis and treatment throughout the course of care, education in resources of cancer diagnosis and treatment, navigation and mobilization of community and governmental resources, therapeutic interventions through counseling services, advance care planning, financial planning, and service as a liaison between the patient, care team, and community agencies.

    Minimum Qualifications :

    Education: Master's in Social Work, required, Graduate degree in a directly related counseling program (ex. ~~~. in Counselor Education) from an accredited college

    Experience: Two years of psychiatric or counseling experience.

    Licensure/Certification: South Carolina Independent Social Work licensure - Current license in Social Work in the state of employment (LISW, LMSW, LCSW), Minimum. BLS (CPR) at hire date, required, or within 90 days of hire.

    Primary Source Verification: SC Labor, Licensing and Regulation (LLR): ~~~- or compact RN state licensing board.

    Knowledge/Skills:

    Knowledge of the specific patient population and the unique needs with which patients present. Experience in working as part of a multi-disciplinary team. Knowledge of how support disciplines are utilized along-side therapeutic disciplines to offer the patient comprehensive service. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records and writing reports. Skill in establishing and maintaining effective working relationships with patients, hospital, medical staff, and the public. Ability to analyze situations and apply critical thinking. Ability to communicate clearly. Ability to react calmly and effectively in emergency situations. Must have a proven track record of excellent communication skills with patients, physicians, and other healthcare providers.

    Contacts : Constant interaction with internal and external customers to include (but not limited to) patients, families, physicians, employees, and visitors.

    Work Demands/Environment :

    Frequent standing, walking, bending, crouching, stooping. Frequent lifting/moving patients. Intermittent lifting, moving, or carrying, pushing, or pulling up to 50 lbs. Frequent use of finger/hand dexterity. Constant reaching with hands/arms. Frequent talking or hearing. Corrected hearing and vision to normal range. Exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Intermittent exposure to risk of exposure to blood-borne pathogens. Moderate noise level in work area. Requires work under stressful conditions, deadlines, or irregular hours. May be required to take call and/or work weekends/holidays based on the needs of the department.
