    Schedule: Monday through Friday either 8am - 5pm or 7:30am - 4pm, rotation of weekends needed then will be off during a week date.

    The community you'll be caring for:

    AdventHealth Parker is a Level II Trauma Center that serves our community by providing excellent, community-focused health care. Since we opened our doors in 2004, we have grown into a regional medical center. In 2017, we received the designation of a Magnet Recognized Hospital. In 2018, we became one of the only five hospitals in the nation, and the only hospital in Colorado, to receive Joint Commission Certification for Bariatrics. We have 170 Licensed beds, 23 Emergency Department private rooms, and 13 surgical suites.

    The role you'll contribute:

    The Clinical Social Worker, Licensed, intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies, and/or require assistance with transitions of care or discharge planning. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations.

    • Applies knowledge of local, state, and federal programs to assist with appropriate referrals.

    • Perform duties and responsibilities in an independent role with oversight of two different cardiac clinics with diverse patient populations.

    • Actively contributes to the social work program supporting patients, families and physicians/providers.

    • Knowledge of community resources, medication patient assistance programs to facilitate optimization of whole patient care and removal of barriers.

    • Independent development of programs to support families or patients dealing with complex medical issues.

    • Promotes individual professional growth and development by meeting the requirements of mandatory/continuing education and supports department-based goals which contribute to the success of the organization.

    • Utilizes effective communication with patients, families, staff, physicians, providers and community leaders to further the development of AHMG and AdventHealth programs.

    • Ensures communication of pertinent patient information among caregivers. Effectively utilizes resources in the development of social work programming in AdventHealth and AHMG.

    • Works collaboratively with leaders to maintain effective and efficient processes.

    • Attends physician/provider and core meetings to facilitate communication and clinical social work input.

    • Communicate effectively with health care team members.

    • Performs other duties as assigned or directed.

    • Facilitates full team discussions including patient and family when ethical dilemmas arise.

    • Social Work Care Manager participates in department educations with topic and content.

    • Develop and establish community and professional relationships that are pertinent to the care and management of high-risk patient populations.

    • Maintains a current working knowledge of services available in the local community, particularly services available to patients with limited or nonexistent resources.

    Qualifications

    The expertise and experiences you'll need to succeed:

    • Masters in Social Work (MSW)

    • Minimum five (5) years experience in hospital/medical social work

    • License Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate (LCSW-A) per state requirement
