    Description:

    Mercy Medical Center is looking for a Licensed Discharge Planner / Social Worker (LSW /LCSW), to join our Case Management Team.

    The Discharge Planner is a licensed medical social worker who works collaboratively with patients, families and the medical team to ensure patients recover and achieve optimal well-being.

    You Will Be Responsible For:

    • Responsible for preforming psychosocial assessments to determine patient care needs

    • Provide ongoing case management support to the patients.

    • Helps connect patients to resources within the community.

    • Vital in assisting and supporting patients who will require discharge planning services.

    • Must have extensive knowledge of community-based resources and programs, managed care, insurance authorization processes, medical terminology, and psychosocial aspects of illness and health.

    Requirements :

    • Education: Bachelor's Degree in Social Work required. Master's Degree Preferred

    • License : Valid social work license from the Commonwealth of Massachusetts Required, minimally at the LSW level Required. LCSW Preferred

    • Experience: Two (2) years of related professional experience required. Fieldwork placements may be substituted in place of one (1) year of experience.
